Depression and mood

Five instruments for low mood. PHQ-9 is the one most clinicians reach for first; EPDS is made for pregnancy and the year after birth; the others go deeper or ask differently.

PHQ-9

PHQ-9 Depression Questionnaire

The Patient Health Questionnaire-9 (PHQ-9) is the gold-standard screening tool for depression used by clinicians worldwide. It measures the frequency and severity of depressive symptoms over the past two weeks.

EPDS

Edinburgh Postnatal Depression Scale (EPDS)

The Edinburgh Postnatal Depression Scale (EPDS) is a 10-question screen for depression during pregnancy and after birth. It is the most widely used perinatal depression screen in the world.

CESD-R

CESD-R Depression Scale

The Center for Epidemiologic Studies Depression Scale Revised (CESD-R) is a 20-item self-report measure designed to assess depressive symptoms in the general population. The revised version aligns with current DSM-5 criteria for major depressive disorder and provides a comprehensive assessment of depressive symptomatology.

HAM-D

Hamilton Depression Rating Scale (HAM-D)

The Hamilton Depression Rating Scale (HAM-D or HDRS) is one of the oldest and most widely used clinician-rated instruments for assessing depression severity. This self-report adaptation allows individuals to estimate the severity of their own depressive symptoms across 17 dimensions.

MADRS

Montgomery–Åsberg Depression Rating Scale (MADRS)

The Montgomery–Åsberg Depression Rating Scale (MADRS) is a 10-item diagnostic questionnaire used to measure the severity of depressive episodes. It was designed to be particularly sensitive to change during treatment, making it a gold standard for measuring antidepressant efficacy in clinical trials.

Anxiety and stress

GAD-7 for worry on its own, HAM-A for how severe anxiety is in body and mind, OCI-R for obsessions and compulsions. DASS-21, set up here for clinicians to give, separates depression, anxiety and stress.

Trauma and dissociation

For when something that happened is still happening — intrusive memories, avoidance, or stretches of time that feel unreal or missing.

Attention and autism

Adult screeners for traits that have usually been there since childhood and were never named.

Personality and traits

Patterns rather than symptoms. These describe tendencies, and none of them diagnoses a personality disorder.

Self-esteem

How you regard yourself overall. Not a diagnosis, but often part of depression and anxiety, and very treatable.

Children and teens

The SCARED, the standard anxiety screener for ages 8 to 18. Two matching versions: one the child answers, one the parent answers about them. Clinicians read both.

Substance use

Ten questions about alcohol and ten about other drugs, each covering the last twelve months.
